<strong>need to increase the dose in order to produce the pharmacological response of equal magnitude<\/strong> and duration<\/p>\n\n\n\n<ul><li>Inability of the subsequent administration of the same dose, of the same drug, to be as effective as its initial dose.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"455\" height=\"273\" with CNS active drugs.<\/li><li>Example &#8211; Tolerance develop to all effects of morphine <strong>except <\/strong><strong>Miosis and Constipation<\/strong><\/li><\/ul>\n\n\n\n<p><strong>Tachyphylaxis<\/strong><\/p>\n\n\n\n<ul><li><strong>Acute development of tolerance<\/strong> after a rapid and repeated administration of a drug at shorter intervals.<\/li><li>May result due to &#8211;<ul><li>Gradual depletion of neurotransmitters from the storage vesicles<\/li><\/ul><ul><li>Change in the sensitivity of the target cells<\/li><\/ul><\/li><li><strong>Tachyphylaxis to nitrates<\/strong><ul><li>Observed among workers in its <strong>manufacturing industry<\/strong><\/li><\/ul><ul><li>On <strong>Monday or Tuesday \u2013 HEADACHE<\/strong> due to <strong>vasodilatory effects of nitroglycerine<\/strong><\/li><\/ul><ul><li><strong>Gradually disappears by Friday<\/strong> due to <strong>development of tolerance<\/strong><\/li><\/ul><ul><li>Headache reappears on Monday after weekend \u2013 <strong>\u201cMonday disease\u201d<\/strong><\/li><\/ul><\/li><\/ul>\n\n\n\n<ul><li><strong>Tolerance v\/s Tachyphylaxis<\/strong><\/li><\/ul>\n\n\n\n<p><strong>Tolerance<\/strong><\/p>\n\n\n\n<ul><li>Observed more often clinically<\/li><li>Develops slowly<\/li><li><strong>Original effect<\/strong> of drug determined lack of sensitivity to a drug<\/li><li>Observed the very first time the drug is administered<\/li><li><strong>Rabbits are tolerant to large doses of Tolerance<\/strong><\/p>\n\n\n\n<ul><li>Metabolic \/ drug dispositional tolerance<\/li><li>Changes in the <strong>distribution or metabolism<\/strong> of a drug <strong>\u2013 given dose produces lower blood concentration than the initial dose<\/strong><strong><\/strong><\/li><li><strong>Higher doses compensates for the losses incurred due to faster drug disposition<\/strong>.<\/li><li><strong>Alcohol<\/strong><\/li><li>It is an example of pharmacokinetic tolerance resulting due to <strong>poor absorption<\/strong><\/li><li>Chronic alcoholics \u2013 <strong>thickened (indurated) gastric mucosa<\/strong> \u2013 reduces the extent of alcohol absorption<\/li><li><strong>Barbiturates<\/strong><\/li><li>It is an example of pharmacokinetic tolerance due to increase in rate of metabolism<\/li><li>They stimulate production of higher levels of hepatic CYPs<\/li><li>Rapid removal of barbiturates from circulation<\/li><li><strong>Nitro vasodilators<\/strong><\/li><li>The pronounced tolerance that occurs results mainly from decreased metabolism<\/li><li>Decreased metabolism \u2013 reduces the release of active metabolite Nitric Oxide<\/li><li><strong>Amphetamine<\/strong><\/li><li>It is an example of pharmacokinetic tolerance due to faster excretion<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image size-large\"><img channel.<\/strong><strong><\/strong><\/li><\/ul>\n\n\n\n<p><strong>Learned Tolerance<\/strong><\/p>\n\n\n\n<ul><li>Reduction in effects of a drug due to compensatory mechanisms acquired by past experiences<\/li><li>It includes \u2013<ul><li>Behavioural tolerance<\/li><\/ul><ul><li>Conditioned tolerance<\/li><\/ul><\/li><\/ul>\n\n\n\n<p><strong>Behavioural Tolerance<\/strong><\/p>\n\n\n\n<ul><li>Refers to the skills that can be developed through repeated experiences with attempting to function despite a state of mild to moderate intoxication<\/li><li>Eg: <strong>Learning to walk a straight line despite the motor impairment produced by alcohol intoxication.<\/strong><\/li><li>Higher levels of intoxication, this tolerance is overcome.<\/li><\/ul>\n\n\n\n<p><strong>Conditioned Tolerance<\/strong><\/p>\n\n\n\n<ul><li>When a <strong>drug affects homeostatic balance, there is usually a reflexive counteraction<\/strong> or adaptation in the direction of maintaining the status quo<\/li><li>Eg: The <strong>blood pressure lowering effect of thiazide diuretics is limited because of gradual activation of renin- angiotensin system<\/strong><strong><\/strong><\/li><li>Drug taken in presence of environmental cues \u2013 begin to predict the effects of the drug and adaptations begins even before the drug reaches its site of action<\/li><li><strong>Prevent the full manifestation of drug\u2019s effects.<\/strong> This mechanism of <strong>conditioned tolerance production follows the classical pavlonian principles of learning<\/strong><\/li><li>Drug received in novel or unexpected circumstances \u2013 conditioned tolerance does not occur<\/li><\/ul>\n\n\n\n<p><strong>Reverse Tolerance<\/strong><\/p>\n\n\n\n<ul><li>Refers <strong>to increase in the response to a drug with repetition of same dose of the drug<\/strong><strong><\/strong><\/li><li>Also known as <strong>SENSITIZATION.<\/strong><\/li><li>It results in a <strong>shift to the left in the dose response curve.<\/strong><\/li><li><strong>Repeated daily administration to rats of cocaine that produces increased motor activity, the effect increases over several days even though the dose remains constant.<\/strong><\/li><\/ul>\n\n\n\n<p><strong>Cross Tolerance<\/strong><\/p>\n\n\n\n<ul><li>Repeated use of a drug in a given category confers tolerance not only to that drug but also to <strong>other drugs in the same structural and mechanistic category<\/strong>.<\/li><li>Those drugs <strong>resembling in chemical structures<\/strong> show cross tolerance.<\/li><li>If a person is tolerant <strong>to morphine,<\/strong> he also shows tolerance <strong>to pethidine<\/strong> (synthetic derivative) and <strong>codeine.<\/strong><\/li><li>Complete cross tolerance is observed in cases like <strong>diazepam and flurazepam<\/strong><\/li><li>Incomplete cross tolerance occurs with the drugs sharing the same pharmacological properties.<\/li><li>Examples include <strong>barbiturates and general anaesthetics<\/strong> &#8211; site of action is CNS, incomplete cross tolerance may be observed although they are not resembling chemically, but having same pharmacological properties.<\/li><li>Sometimes cross tolerance occurs between two drugs that do not share mechanisms of action or classification<\/li><li>Eg: Alcohol exhibit cross tolerance with nicotine in animal models \u2013 possible in humans<\/li><li>Amphetamine exhibit cross tolerance with caffeine<\/li><\/ul>\n\n\n\n<p><strong>Clinical implications of tolerance<\/strong><\/p>\n\n\n\n<ul><li><strong>Regular use of inhaled \u03b2-agonist<\/strong> leads to tolerance to its bronchoprotective effect.<\/li><li>Eg- Salbutamol , salmeterol<\/li><li>Increase the dose of salbutamol<\/li><li>Combining salbutamol with ipratropium bromide<\/li><\/ul>\n\n\n\n<ul><li><strong>Tachyphylaxis to nitrates<\/strong><\/li><li>Generation of tachyphylaxis can be suppressed or even avoided by supplementation of appropriate antioxidants (SOD, vitamin C)<\/li><li>Even fully developed tolerance (during non-intermittent GTN-administration) could be reversed by starting an appropriate antioxidant supplementation.<\/li><\/ul>\n\n\n\n<ul><li>Tolerance leads to treatment failure and progression of disease<\/li><li>Prompt early recognition and appropriate alternative drug treatment helps patients<\/li><li>Abused drugs \u2013 can lead to fatal overdoses due to tolerance<\/li><\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Drug
