{"id":479,"date":"2020-02-16T12:15:31","date_gmt":"2020-02-16T06:45:31","guid":{"rendered":"https:\/\/medicineplexus.com\/?p=479"},"modified":"2020-02-16T12:15:31","modified_gmt":"2020-02-16T06:45:31","slug":"fibrinolytics-thrombolytics","status":"publish","type":"post","link":"https:\/\/medicineplexus.com\/fibrinolytics-thrombolytics\/","title":{"rendered":"FIBRINOLYTICS (Thrombolytics)"},"content":{"rendered":"\n<p><strong>FIBRINOLYTICS (Thrombolytics)<\/strong><\/p>\n\n\n\n<ul><li>These are drugs used to lyse thrombi\/clot to recanalize occluded blood vessels (mainly coronary artery).<\/li><li>They are therapeutic rather than prophylactic and work by activating the natural fibrinolytic system<\/li><li>Pathophysiology \u00e0 Hemostatic plug of platelets formed at the site of injury to blood vessels is reinforced by fibrin deposition to form a thrombus.<\/li><li>Once repair is over, the fibrinolytic system is activated to remove the fibrin.<\/li><li>The enzyme responsible for digesting fibrin \u00e0 Plasmin generated from plasminogen by tissue plasminogen activator (t-PA), which is produced primarily by vascular endothelium.<\/li><li>The t-PA selectively activates fibrin-bound plasminogen within the thrombus, and any plasmin that leaks is inactivated by circulating antiplasmins.<\/li><li>Thus, activation of circulating plasminogen induces a lytic state whose major complication is hemorrhage.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"618\" height=\"305\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-116.png?w=618\" alt=\"\" class=\"wp-image-480\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-116.png 618w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-116-300x148.png 300w\" sizes=\"(max-width: 618px) 100vw, 618px\" \/><\/figure>\n\n\n\n<ul><li>In general, venous thrombi are lysed more easily by fibrinolytics than arterial, and recent thrombi respond better.<\/li><li>They have little effect on thrombi &gt; 3 days old.<\/li><li>The clinically important fibrinolytics are:<ul><li>Streptokinase<\/li><\/ul><ul><li>Urokinase<\/li><\/ul><ul><li>Alteplase (rt-PA)<\/li><\/ul><ul><li>Reteplase<\/li><\/ul><ul><li>Tenecteplase<\/li><\/ul><\/li><\/ul>\n\n\n\n<p><strong>Streptokinase<\/strong><\/p>\n\n\n\n<ul><li>Obtained from <strong>\u03b2 haemolytic Streptococci group C,<\/strong><\/li><li>it is the first fibrinolytic drug to be used clinically, but is not employed now except for considerations of cost.<\/li><li>Streptokinase is inactive as such; combines with circulating plasminogen molecules to form an activator complex which then causes limited proteolysis of other plasminogen molecules to generate the active enzyme plasmin.<\/li><li>Stk. is <strong>non-fibrin specific, i.e. activates both circulating as well as fibrin bound plasminogen<\/strong>.<\/li><li>Therefore, it depletes circulating fibrinogen and predisposes to bleeding.<\/li><li>Compared to newer more fibrin-specific tissue plasminogen activators (alteplase, etc.) it is <strong>less effective in opening occluded coronary arteries, and causes less reduction in MI related mortality.<\/strong><\/li><li>other disadvantages \u00e0 <strong>Antistreptococcal antibodies due to past infections inactivate considerable fraction of the initial dose of Stk<\/strong> -\u00e0 A loading dose therefore is necessary.<\/li><li>Stk is antigenic\u2014can cause <strong>hypersensitivity reactions; anaphylaxis occurs in 1\u20132% patients<\/strong>.<\/li><li>It <strong>cannot be used second time due to neutralization by antibodies <\/strong>generated in response to the earlier dose.<\/li><\/ul>\n\n\n\n<p>For MI: 7.5\u201315 lac IU infused i.v. over 1 hr.<\/p>\n\n\n\n<p><strong>Urokinase<\/strong><\/p>\n\n\n\n<ul><li>enzyme isolated from human urine; but <strong>commercially prepared from cultured human kidney cells.<\/strong><\/li><li>activates plasminogen directly<\/li><li>nonantigenic<\/li><li>Indicated in patients in whom streptokinase has been given for an earlier episode, but is seldom used now.<\/li><\/ul>\n\n\n\n<p><strong>Alteplase<\/strong> <strong>(recombinant tissue plasminogen activator (rt-PA)<\/strong><\/p>\n\n\n\n<ul><li><strong>moderately specific<\/strong> for fibrin-bound plasminogen, so that <strong>circulating fibrinogen is lowered only by ~ 50%.<\/strong><\/li><li>It is rapidly cleared by liver and inactivated by plasminogen activator inhibitor-1 (PAI-1).<\/li><li>The plasma t\u00bd is 4\u20138 min. Because of the short t\u00bd, it needs to be given by slow i.v. infusion and often requires heparin coadministration.<\/li><li>nonantigenic, but expensive.<\/li><\/ul>\n\n\n\n<p><strong>Reteplase<\/strong><\/p>\n\n\n\n<ul><li>modified form of rt-PA that is longer acting, but somewhat less specific for fibrin-bound plasminogen.<\/li><li>The longer duration of action enables bolus dose administration (10 mg over 10 min repeated after 30 min).<\/li><\/ul>\n\n\n\n<p><strong>Tenecteplase \u2013 rt-PA<\/strong><\/p>\n\n\n\n<ul><li>higher fibrin selectivity, slower plasma clearance (longer duration of action) and resistance to inhibition by PAI-1.<\/li><li><strong>only fibrinolytic agent that can be injected i.v. as a single bolus dose over 10 sec<\/strong>, while alteplase requires 90 min infusion.<\/li><li><strong>This feature makes it possible to institute fibrinolytic therapy immediately on diagnosis of ST segment elevation myocardial infarction (STEMI), even during transport of the patient to the hospital.<\/strong><\/li><li>Risk of noncerebral bleeding may be lower with tenecteplase, but cranial bleeding incidence is similar.<\/li><\/ul>\n\n\n\n<p><strong>Uses of fibrinolytics<\/strong><\/p>\n\n\n\n<ol type=\"1\"><li><strong>Acute myocardial infarction<\/strong><ol><li>Chief indication \u00e0 alternative first line approach to emergency percutaneous coronary intervention (PCI) with stent placement.<\/li><\/ol><\/li><li><strong>Thrombolysis may be favoured if it can be started within 1\u20132 hours of onset. After 3 hours, PCI is favoured.<\/strong><\/li><li>Recanalization of thrombosed coronary artery has been achieved in 50\u201390% cases.<\/li><li>Time lag in starting the infusion is critical for reducing area of necrosis, preserving ventricular function and reducing mortality.<\/li><li>Aspirin with or without heparin is generally started concurrently or soon after thrombolysis to prevent re-occlusion.<\/li><li>Alteplase has advantages over streptokinase, including higher thrombolytic efficacy. However, incidence of haemorrhage is not lower.<\/li><\/ol>\n\n\n\n<ul><li><strong>Deep vein thrombosis in leg, pelvis, shoulder etc<\/strong><ul><li>up to 60% patients can be successfully treated.<\/li><\/ul><ul><li>Thrombolytics can decrease subsequent pain and swelling, but the main advantage is preservation of venous valves and may be a reduced risk of pulmonary embolism, though at the risk of haemorrhage.<\/li><\/ul><\/li><\/ul>\n\n\n\n<ul><li><strong>Pulmonary embolism<\/strong><ul><li>Fibrinolytic therapy is indicated in large, life-threatening PE.<\/li><\/ul><ul><li>The lung function may be better preserved, but reduction in mortality is not established.<\/li><\/ul><\/li><\/ul>\n\n\n\n<ul><li><strong>Peripheral arterial occlusion<\/strong><ul><li>Fibrinolytics re-canalise ~40% limb artery occlusions, especially those treated within 72 hr.<\/li><\/ul><ul><li>However, it is indicated only when surgical thrombectomy is not possible.<\/li><\/ul><ul><li>Regional intraarterial fibrinolytics have been used for limb arteries with greater success.<\/li><\/ul><ul><li>Peripheral arterial thrombolysis is followed by short-term heparin and long-term aspirin therapy.<\/li><\/ul><\/li><\/ul>\n\n\n\n<ul><li><strong>Stroke:<\/strong><ul><li>Thrombolytic therapy of ischaemic stroke is controversial.<\/li><\/ul><ul><li>Possibility of improved neurological outcome is to be balanced with risk of intracranial haemorrhage.<\/li><\/ul><ul><li>However, alteplase is approved for use in ischaemic stroke, and current opinion strongly recommends use of i.v. alteplase in carefully selected patients who can be treated within 3 hours of onset, and in whom intracranial haemorrhage is ruled out along with all risk factors for bleeding<\/li><\/ul><\/li><\/ul>\n\n\n\n<p><strong>Contraindications<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"566\" height=\"256\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-117.png?w=566\" alt=\"\" class=\"wp-image-481\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-117.png 566w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-117-300x136.png 300w\" sizes=\"(max-width: 566px) 100vw, 566px\" \/><\/figure>\n\n\n\n<p><strong>ANTIFIBRINOLYTIC DRUGS<\/strong><\/p>\n\n\n\n<ul><li>drugs which <strong>inhibit plasminogen activation and dissolution of clot, and<\/strong><\/li><li><strong>are used to check fibrinolysis associated bleeding<\/strong>.<\/li><\/ul>\n\n\n\n<p><strong>Epsilon amino-caproic acid (EACA)<\/strong><\/p>\n\n\n\n<ul><li>It is a lysine analogue<\/li><li>combines with the lysine binding sites of plasminogen and plasmin so that the latter is not able to bind to fibrin and lyse it.<\/li><li>specific antidote for fibrinolytic agents<\/li><li>primary indication is to counteract the effect of fibrinolytic drugs and bleeding due to their use.<\/li><li>In haemophiliacs, it has adjunctive value for controlling bleeding due to tooth extraction, prostatectomy, trauma, etc.<\/li><li>s\/e \u00e0 intravascular thrombosis. Rapid i.v. injection results in hypotension, bradycardia and may be arrhythmias. It should be used cautiously when renal function is impaired. Myopathy occurs rarely.<\/li><\/ul>\n\n\n\n<p>The large dose needed is a limitation, <strong>and tranexamic acid is mostly preferred.<\/strong><\/p>\n\n\n\n<p><strong>Tranexamic acid<\/strong><\/p>\n\n\n\n<ul><li>Like EACA, it binds to the lysine binding site on plasminogen and prevents its combination with fibrin leading to fibrinolysis.<\/li><li>7 times more potent than EACA, and is preferred for prevention\/control of excessive bleeding due to:<\/li><\/ul>\n\n\n\n<p>\u2022 Fibrinolytic drugs.<\/p>\n\n\n\n<p>\u2022 Cardio-pulmonary bypass surgery.<\/p>\n\n\n\n<p>\u2022 Tonsillectomy, prostatic surgery, tooth extraction in haemophiliacs.<\/p>\n\n\n\n<p>\u2022 Menorrhagia, especially due to IUCD.<\/p>\n\n\n\n<p>\u2022 Recurrent epistaxis, hyphema due to ocular trauma, peptic ulcer.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>FIBRINOLYTICS (Thrombolytics) These are drugs used to lyse thrombi\/clot to recanalize occluded blood vessels (mainly coronary artery).
