{"id":502,"date":"2020-02-16T12:58:22","date_gmt":"2020-02-16T07:28:22","guid":{"rendered":"https:\/\/medicineplexus.com\/?p=502"},"modified":"2020-02-16T12:58:22","modified_gmt":"2020-02-16T07:28:22","slug":"glaucoma","status":"publish","type":"post","link":"https:\/\/medicineplexus.com\/glaucoma\/","title":{"rendered":"Glaucoma"},"content":{"rendered":"\n<p><strong>Glaucoma<\/strong><\/p>\n\n\n\n<ul><li>Glaucoma is the second leading cause of blindness in the world.<\/li><li>Glaucoma is characterized by progressive damage to optic nerve associated with raised intraocular pressure (&gt; 21 mm Hg).<\/li><li>The term &#8220;ocular hypertension&#8221; is used for people with consistently raised intraocular pressure&nbsp;(IOP) without any associated optic nerve damage.<\/li><li><strong>Normal tension glaucoma<\/strong>: Optic disc changes despite normal IOP<\/li><li><strong>Primary Open Angle Glaucoma (POAG):<\/strong> Patency of trabecular meshwork affected<\/li><li><strong>Primary Angle Closure Glaucoma (PACG):<\/strong> Shallow ant. Chamber, narrow iridiocorneal angle<\/li><li>Raised intraocular pressure which normally ranges from 10-21 mm of Hg is the most important and only modifiable risk factor for glaucoma and IOP-lowering medications comprise the primary treatment strategy.<\/li><li>Rise in intraocular tension is either due to excessive production or due to less drainage of aqueous humor.<\/li><li>Lowering of i.o.t. retards progression of optic nerve damage even in normal\/low i.o.t. glaucoma<\/li><\/ul>\n\n\n\n<p><strong>Pathophysiology<\/strong><\/p>\n\n\n\n<ul><li>\u2191IOP &#8211; glaucomatous optic neuropathy involving retinal ganglion cell death d\/t apoptosis.<\/li><li>Characterized by changes in the optic disc and irreversible visual field defects.<\/li><li>Optic disc changes: a) Neuroretinal rim thinning<\/li><\/ul>\n\n\n\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; b) Pallor &amp;<\/p>\n\n\n\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; c) Progressive optic disc cupping<\/p>\n\n\n\n<ul><li>Visual field defects detected only after 40% of the axons are lost.<\/li><li>Primary factors :<ul><li>\u2191IOP<\/li><\/ul><ul><li>Vascular dysregulation<\/li><\/ul><\/li><li>Secondary factors:<ul><li>Excitotoxic damage caused by glutamate or glycine released from injured neurons<\/li><\/ul><ul><li>Oxidative damage caused by over-production of nitric oxide (NO) &amp; ROS.<\/li><\/ul><\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"400\" height=\"231\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-126.png?w=400\" alt=\"\" class=\"wp-image-503\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-126.png 400w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-126-300x173.png 300w\" sizes=\"(max-width: 400px) 100vw, 400px\" \/><\/figure>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"634\" height=\"301\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-127.png?w=634\" alt=\"\" class=\"wp-image-504\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-127.png 634w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-127-300x142.png 300w\" sizes=\"(max-width: 634px) 100vw, 634px\" \/><\/figure>\n\n\n\n<p>Major amount of aqueous (- 90%) drains through the trabecular route, while ~ I 0% fluid passes into the connective tissue spaces within the ciliary muscle- then via suprachoroidal into episcleral vessels (uveoscleral outflow).<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"495\" height=\"410\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-128.png?w=495\" alt=\"\" class=\"wp-image-505\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-128.png 495w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-128-300x248.png 300w\" sizes=\"(max-width: 495px) 100vw, 495px\" \/><\/figure>\n\n\n\n<ul><li><strong>Site of action of miotics in angle closure glaucoma<\/strong>: contraction of sphincter pupillae removes pupillary block and reverses obliteration of iridocorneal angle<\/li><\/ul>\n\n\n\n<ul><li><strong>Site of action of miotics in open angle glaucoma:<\/strong> contraction of ciliary muscle pulls on scleral spur and improves trabecular patency<\/li><\/ul>\n\n\n\n<ul><li><strong>Site of action of (a) beta blockers (b) a1 agonists (c) a2 agonists (d) carbonic anhydrase inhibitors:<\/strong> all reduce aqueous secretion by ciliary body<\/li><\/ul>\n\n\n\n<ul><li><strong>Site of action of prostaglandins and adrenaline (alpha agonist action):<\/strong> increase uveoscleral outflow by altering permeability and\/or pressure gradients<\/li><\/ul>\n\n\n\n<ul><li><strong>Site of action of adrenaline (beta2 agonist action):<\/strong> possibly increases aqueous conductivity of trabecular filtering cells<\/li><\/ul>\n\n\n\n<p><strong>Drugs useful in <\/strong><strong>primary open angle glaucoma (POAG) <\/strong><strong>are:<\/strong><\/p>\n\n\n\n<p>Ocular hypotensive drugs are used on a long-term basis and constitute the definitive treatment in majority of cases.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"350\" height=\"353\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-129.png?w=350\" alt=\"\" class=\"wp-image-506\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-129.png 350w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-129-297x300.png 297w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-129-150x150.png 150w\" sizes=\"(max-width: 350px) 100vw, 350px\" \/><\/figure>\n\n\n\n<p><strong>Prostaglandin analogs:<\/strong><\/p>\n\n\n\n<ul><li>PGF2\u03b1 increases uveoscleral outflow.<\/li><li>Latanoprost (i.o.t. reduction by 25-35%), bimatoprost and unoprostone are PGF2\u03b1 derivatives useful in glaucoma.<\/li><li>It reduces i.o.t. in normal pressure glaucoma also.<\/li><li>good efficacy, once daily application and absence of systemic complications\u00e0 drug of choice for POAG.<\/li><li>Bimatoprost \u00e0 s\/e growth of eyelashes \u00e0 can be utilized for treatment of hypotrichosis.<\/li><li>Though ocular irritation and pain are relatively frequent, no systemic side effects are reported. Blurring of vision, increased iris pigmentation, thickening and darkening of eyelashes have occurred in some cases. Macular edema can develop during treatment with any PGF20 analogue, especially in aphakic patients; a watch should be kept to-detect it early.<\/li><\/ul>\n\n\n\n<ul><li><strong>Travoprost <\/strong>&#8211; i.o.t. lowering efficacy and side effects are comparable to latanoprost.<\/li><\/ul>\n\n\n\n<p><strong>\u03b2<\/strong><strong>-blockers:<\/strong><\/p>\n\n\n\n<ul><li>among the first line drugs for POAG.<\/li><li>Ciliary processes contain \u03b22 (vasodilatory) and \u03b12 (vasoconstrictor) receptors.<\/li><li>Whenever vasodilation occurs, amount of blood reaching in the ciliary body increases resulting in excessive secretion of aqueous humor.<\/li><li>Therefore, \u03b2-blockers and \u03b1 agonists can decrease the secretion of aqueous.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"338\" height=\"244\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-130.png?w=338\" alt=\"\" class=\"wp-image-507\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-130.png 338w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-130-300x217.png 300w\" sizes=\"(max-width: 338px) 100vw, 338px\" \/><\/figure>\n\n\n\n<ul><li>\u03b11 adrenoceptors constrict ciliary vessels and reduce aqueous production: are activated by adrenaline.<\/li><li>\u03b12 adrenoceptors located on ciliary epithelium reduce aqueous secretion: are activated by adrenaline\/apraclonidine\/brimonidine.<\/li><li>\u03b23 adrenoceptors located on ciliary epithelium enhance aqueous secretion via increased cAMP. Their blockade by timolol\/betaxolol reduces secretion.<\/li><li>Carbonic anhydrase present within ciliary epithelial cells generates HCO3-; ion which is secreted into aqueous humor. Acetazolamide\/dorzolamide inhibit it and reduce<\/li><\/ul>\n\n\n\n<p>aqueous secretion.<\/p>\n\n\n\n<ul><li>Timolol, betaxolol, levobetaxolol, levobunolol, carteolol and metipranolol have been approved for use in glaucoma.<\/li><li>Levobunolol is longest acting whereas betaxolol is cardioselective (therefore less efficacious but safe in asthmatics) \u03b2-blocker.<\/li><li>20-35% fall in i.o.t.<\/li><li><strong>Ocular side effects<\/strong> of beta blockers \u00e0 generally mild and infrequent- stinging, redness and dryness of eye, corneal hypoesthesia, allergic blepharoconjunctivitis and blurred vision.<\/li><li><strong>Systemic adverse effects<\/strong> \u00e0 major limitations \u00e0 occur due to absorption through nasolacrimal duct \u00e0 life threatening bronchospasm in asthmatic and COPD patients.<\/li><\/ul>\n\n\n\n<p>Bradycardia, accentuation of heart block and CHF are likely to occur especially in the elderly.<\/p>\n\n\n\n<p>Systemic adverse effects can be minimized by applying mild pressure on the inner canthus of the eye for about 5 min. after instilling the eye drop to prevent entry of the drug into nasolacrimal duct from where it is mainly absorbed.<\/p>\n\n\n\n<p><strong>Betaxolol<\/strong> \u00e0 beta1 selective blocker \u00e0less bronchopulmonary and probably less cardiac, central and metabolic side effects.<\/p>\n\n\n\n<p>In addition, it appears to exert a <strong>protective effect on retinal neurons independent of i.o.t. lowering, <\/strong>by blocking some Ca2+ channels and reducing Na+\/Ca2 influx.<\/p>\n\n\n\n<p><strong>Advantages of topical beta blockers over miotics<\/strong><\/p>\n\n\n\n<ul><li>No change in pupil size: no diminution of vision in dim light and in patients with cataract<\/li><li>No induced myopia which is especially troublesome in young patients<\/li><li>No headache\/brow pain due to persistent spasm of iris and ciliary muscles<\/li><li>No fluctuations in i.o.t. as occur with pilocarpine drops<\/li><li>Convenient twice\/once daily application sufficient<\/li><\/ul>\n\n\n\n<p><strong>\u03b1<\/strong><strong>-Agonists:<\/strong><\/p>\n\n\n\n<ul><li>Dipivefrine (prodrug of adrenaline) and adrenaline act by increasing trabecular outflow whereas<\/li><li>apraclonidine (lowers i.o.t. by ~25%.)and brimonidine (selective a2 agonists) act by decreasing aqueous secretion.<\/li><li>Apraclonidine can cause lid retraction, itching, lid dermatitis, follicular conjunctivitis, mydriasis, dryness of mouth and nose<\/li><li>brimonidine (lowers i.o.t. by 20- 27%)is associated with anterior uveitis. Allergic conjunctivitis and other ocular side effects are similar to but less frequent than with apraclonidine.<\/li><li>Both of these can cause drowsiness.<\/li><\/ul>\n\n\n\n<ul><li>Dipivefrine can cause cystoid macular edema in aphakics.<\/li><\/ul>\n\n\n\n<p><strong>Carbonic anhydrase inhibitors:<\/strong><\/p>\n\n\n\n<ul><li>Acetazolamide (oral), brinzolamide and dorzolamide (both topical) act by decreasing the secretion of aqueous humor<\/li><li>Oral treatment with acetazolamide (0.25 g 6-12 hourly) reduces aqueous formation by limiting generation of bicarbonate ion in the ciliary epithelium<\/li><li>used to supplement ocular hypotensive drugs for short-term indications like angle closure, before and after ocular surgery\/laser therapy<\/li><li><strong>Dorzolamide &#8211; <\/strong>lowers i.o.t. by ~20%; Ocular stinging, burning, itching, corneal edema and bitter taste are the usual side effects.<\/li><\/ul>\n\n\n\n<p><strong>Miotics:<\/strong><\/p>\n\n\n\n<ul><li>Pilocarpine (directly acting cholinomimetic) and physostigmine (indirectly acting cholinomimetic) increase aqueous outflow by causing miosis.<\/li><li>Pilocarpine is short acting, therefore requires frequent daily dosing.<\/li><li>Demacarium and ecothiophate (both are long acting cholinomimetics) \u00e0 rarely used \u00e0 accelerate cataract development.<\/li><\/ul>\n\n\n\n<p><strong>For closed-angle glaucoma<\/strong>,<\/p>\n\n\n\n<ul><li>occurs in individuals with a narrow iridocorneal angle and shallow anterior chamber. The i.o.t. remains normal until an attack is precipitated, usually by mydriasis<\/li><li>The i.o.t. rises rapidly to very high values (40- 60 mmHg).<\/li><li>Vigorous therapy employing several measures to reduce i.o.t. is instituted.<\/li><\/ul>\n\n\n\n<ul><li><strong>Hypertonic mannitol (20%) 1.5- 2 g\/kg or glycerol (10%):<\/strong><\/li><\/ul>\n\n\n\n<p>infused i.v. decongest the eye by osmotic action.<\/p>\n\n\n\n<ul><li><strong>Acetazolamide: 0.5 g i.v.<\/strong> followed by oral twice daily is started concurrently.<\/li><\/ul>\n\n\n\n<ul><li><strong>Miotic:<\/strong> Once the i.o.t. starts falling due to the above i.v. therapy, pilocarpine 1-4% is instilled every 10 min initially and then at longer intervals.<\/li><\/ul>\n\n\n\n<p>Pupillary block is removed and iridocorneal angle is freed<\/p>\n\n\n\n<ul><li><strong>Topical beta blocker:<\/strong> Timolol 0.5% is instilled 12 hourly in addition.<\/li><\/ul>\n\n\n\n<ul><li><strong>Apraclonidine ( 1 %)\/latanoprost 0.005%<\/strong> instillation may be added.<\/li><li>Drugs are used only to terminate the attack of angle closure glaucoma.<\/li><li>definitive treatment is surgery \u00e0 Laser peripheral iridotomy or surgical peripheral iridectomy.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"646\" height=\"484\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-131.png?w=646\" alt=\"\" class=\"wp-image-508\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-131.png 646w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-131-300x225.png 300w\" sizes=\"(max-width: 646px) 100vw, 646px\" \/><figcaption><br><\/figcaption><\/figure>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"729\" height=\"812\" src=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-132.png?w=729\" alt=\"\" class=\"wp-image-509\" srcset=\"https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-132.png 729w, https:\/\/medicineplexus.com\/wp-content\/uploads\/2020\/02\/image-132-269x300.png 269w\" sizes=\"(max-width: 729px) 100vw, 729px\" \/><\/figure>\n\n\n\n<p><strong>Why new drugs?<\/strong><\/p>\n\n\n\n<ul><li>Neural damage irreversible \u2013 <strong>need for neuroprotective agents<\/strong><\/li><li>Patients with asthma, bradycardia, cataract, allergy to sulfa drugs or topical brimonidine \u2013 not much options left other than SX<\/li><li>Need for preservative free drugs<ul><li>Benzalkonium \u2013 punctate \/ ulcerative keratopathy<\/li><\/ul><ul><li>Thiomersal \u2013 hypersensitivity<\/li><\/ul><\/li><li>Drugs for newer drug delivery systems<\/li><\/ul>\n\n\n\n<ol type=\"1\"><li>&nbsp;<strong>lanoprostene bunod<\/strong><\/li><li>FDA approved in November 2017<\/li><li>releases nitric oxide\u00e0 relax the trabecular meshwork to increase drainage of aqueous humor and improve aqueous humor outflow.<\/li><li><strong>first nitric oxide-containing medication for glaucoma<\/strong> treatment worldwide.<\/li><\/ol>\n\n\n\n<ul><li>Clinical trials \u00e0 comparing <strong>lanoprostene<\/strong> to latanoprost and timolol \u00e0 greater eye pressure lowering compared to latanoprost and timolol.<\/li><\/ul>\n\n\n\n<ul><li>Side effects \u00e0 similar to that of prostaglandin analogues<\/li><\/ul>\n\n\n\n<ul><li>Common side effects \u00e0 redness of the conjunctiva, eyelash growth, eye irritation, and eye pain at the time of instilling the medication<\/li><\/ul>\n\n\n\n<ul><li><strong>Netarsudil<\/strong><\/li><li>rho-associated protein kinase inhibitors, or ROCK inhibitors.<\/li><li>approved by the FDA in December 2017<\/li><li>ROCK \u00e0 control cellular structures involved in cell shape, motility, contractility, proliferation, and apoptosis.<\/li><li>ROCK inhibitors \u00e0 lower eye pressure by relaxing the trabecular meshwork and connection passageway, called Schlemm\u2019s canal.<\/li><li>This canal drains the aqueous humor from the anterior chamber of the eye into the veins draining the eyeball.<\/li><li>this decreases the resistance to drainage or outflow in the conventional outflow pathway, thus lowering eye pressure.<\/li><li>some preliminary laboratory \u00e0 role in promoting survival of the retinal ganglion cells \u00e0 mechanism has not yet proven<\/li><\/ul>\n\n\n\n<ul><li>Clinical trials comparing <strong>Netarsudil <\/strong>to latanoprost and timolol \u00e0 lowers eye pressure to a similar degree.<\/li><\/ul>\n\n\n\n<ul><li>The most frequent side effects include eye redness, small bleeds in the conjunctiva, and corneal changes. Rhopressa is dosed twice daily<\/li><\/ul>\n\n\n\n<ul><li><strong>Ripasudil <\/strong>&#8211; Rho-associated protein kinase (ROCK) inhibitor &#8211; Approved in Japan in September 2014 and in commercial use in Japan<\/li><li><strong>Netarsudil + Latanoprost &#8211; <\/strong>ROCK and norepinephrine transporter inhibitor + prostaglandin analogue &#8211; Phase III trials completed<\/li><\/ul>\n\n\n\n<ul><li><strong>Trabodenoson &#8211; Adenosine receptor agonists<\/strong> &#8211; <strong><\/strong><\/li><li>Adenosine receptor agonists \u00e0 <strong>stimulate secretion of matrix metalloproteinases (MMPs)<\/strong> in the endothelial cells lining the trabecular meshwork. <strong><\/strong><\/li><li>This causes cell volume shrinkage and extracellular matrix remodeling \u00e0 facilitates conventional aqueous outflow.[<strong><\/strong><\/li><li><strong>Trabodenoson<\/strong> \u00e0 Phase III clinical trial completed<strong><\/strong><\/li><li>Phase II trials demonstrated a median IOP reduction of \u22126.5 mmg \u00b1 2.5 mmHg at 500 mcg dose by day 28 of trial. <strong><\/strong><\/li><\/ul>\n\n\n\n<ul><li><strong>Prostanoid receptor agonists<\/strong><\/li><li>latanoprost and travoprost, act on prostanoid prostaglandin F receptor (FP receptor), a receptor for Prostaglandin F2 alpha (PGF2\u03b1). <strong><\/strong><\/li><li>Bimatoprost is a synthetic PGF2\u03b1 mimetic, termed prostamide F2\u03b1. <strong><\/strong><\/li><li>These drugs enhance aqueous outflow through the uveoscleral pathway and cause IOP lowering.<strong><\/strong><\/li><\/ul>\n\n\n\n<ul><li>Recently, EP2, EP3 receptors \u00e0 <strong>DE\u2011117<\/strong> \u00e0 an EP2 agonist. <strong><\/strong><\/li><\/ul>\n\n\n\n<ul><li>cause relaxation of endothelial cells in the Schlemm\u2019s canal, facilitating uveoscleral outflow.<strong><\/strong><\/li><\/ul>\n\n\n\n<ul><li>also increase conventional outflow by acting on the trabecular meshwork, decreasing cell contractility and collagen deposition<strong><\/strong><\/li><\/ul>\n\n\n\n<ul><li><strong>Taprenepag isopropyl<\/strong> is an EP2 agonist.
