This is simply not possible.<\/li><li><strong>Focussed screening : <\/strong>A limited number of compounds are pre-selected for screening. Has proved successful as a hit generation strategy. Useful when 3D structure of target is known (e.g. crystal structure of a receptor<\/li><li>most drug discovery labs \u00e0 library collection of 400,000 to 1 million or more compounds.<\/li><li>Primary screen is designed to rapidly identify hits from compound libraries<\/li><li>Primary screens are run in multiplets of single compound concentrations. Hits are then retested, usually independently from the first assay.<\/li><li>If a compound exhibits the same activity, it is coined as <strong>confirmed hit<\/strong>, which proceeds to secondary screens or lead optimization<\/li><li>Primary screen is designed to rapidly identify hits from compound libraries<\/li><li>allows potential drugs to be quickly and efficiently screened to find candidates that should be explored in more detail<\/li><li><strong><\/strong><\/li><li><strong>Disadvantage- <\/strong>Need for relatively pure products \u00e0 High cost<\/li><li><strong>Advantage &#8211; <\/strong>Many types of in vitro assays can be readily converted to HTS.<\/li><\/ul>\n\n\n\n<p><strong>Screening drug for genetic disorders<\/strong><strong><\/strong><\/p>\n\n\n\n<ul><li>Bioinformatics technology allows researchers to analyze the tetra bytes of data produced by the Human Genome Project.<\/li><li>Gene sequence databases, gene expression databases, protein sequence databases, and related analysis tools all help to determine whether and how a particular molecule is directly involved in a disease process and in turn helps find new and better drug targets.<\/li><\/ul>\n\n\n\n<p><strong>Pharmacometrics<\/strong><strong><\/strong><\/p>\n\n\n\n<p>Science of developing and applying mathematical and statistical methods to:<\/p>\n\n\n\n<ul><li>a. Characterize, understand and predict a drug\u2019s pharmacokinetic and pharmacodynamic behavior<\/li><li>b. Quantify uncertainty of information about that behavior<\/li><li>c. Rationalize data-driven decision making in the drug development process &amp; pharmacotherapy<\/li><\/ul>\n\n\n\n<p><strong>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; PK\/PD modeling \u2013 application<\/strong><\/p>\n\n\n\n<ul><li>PK\/PD assists in identification of appropriate animal model, later in Phase I they assist in PK, safety and tolerability of drug candidate<\/li><li>Example \u2013 How PK\/PD model helps in dose selection for clinical trial<\/li><li>The researchers are developing a new drug, during research they have collected lot of data \u2026during early phase of development and now have a whole pile of data to work with<\/li><li>But it is still challenging for the researchers why some pt respond differently to others<\/li><li>PK \u2013 what body does to the drug, PD &#8211;&nbsp; what the drug does to the body. This much we already know but how this will help to answer the researchers question<\/li><li>Pt. are individual with diff characteristics\u00e0 so even though they get the same dose, the drug conc. Will vary \u00e0 body weight, gender, age, drug interactions, metabolism, \u00e0 <strong>reason for PK variability<\/strong><\/li><li>The pharmacologist uses a PK model together with the collected data to quantify PK variability<\/li><li>Patient suffering from the same disease may respond diff. to the drug conc.<\/li><li>Drug acts on the target \u00e0 which maybe receptor or enzyme, diff. factors such as no. of receptors, the time span in which receptors regenerate themselves or their diff. functionality \u00e0 <strong>reasons for PD variability<\/strong><\/li><li>PK\/PD model \u00e0 helps to understand how drug effects relates to conc. Hence the concept of e consent have arised. The FDA proposed&nbsp;the following definition for \u201ce-Consent\u201d in its March 2015 guidance:&nbsp;<\/li><li>\u201celectronic informed consent refers to using electronic systems and processes that may employ multiple electronic media (e.g., text, graphics, audio, video) to convey information related to the study and to obtain and document informed consent.<\/li><li>the consent is designed to explain the clinical trial process clearly using terminology that is easy to understand by a layperson<\/li><li>Conventional consent form- 15 to 20 page document<\/li><li>With an e-consent, the patient will have the opportunity to be informed by different means, such as movies, clips, graphics, a multi media interaction presented on the subjects own device<\/li><li>allow the subject the take the device home, discuss with family and friends and even do some additional research all from the same device without any additional needs<\/li><li>The FDA guidance \uf0e0 very supportive of an e-Consent process, \uf0e0ability to remotely update the informed consent when amendments are implemented and new information becomes available during the conduct of a trial<\/li><li>&nbsp;According to a survey conducted in 146 respondents from 100 biotech, pharmaceutical, CRO, and IRB organizations. \u00e0 high cost, regulatory concerns are preventing many organizations from becoming early adopters of eConsent&nbsp;<\/li><\/ul>\n\n\n\n<p><strong>Electronic CRF<\/strong><\/p>\n\n\n\n<ul><li>Paper CRF is the traditional way of data capture &#8211; better option if studies are small or vary in design<\/li><li>In the current global scenario, eCRFs are preferred over paper CRFs<\/li><li>encourage the sponsor\/pharmaceutical company to carry out large multicentric studies at the same time due to the ease of administration<\/li><li>ensures no duplication of CRF pages \uf0e0 protocol ID, site code, subject ID, and patient initials will be generated by the system automatically<\/li><li>Chances of error during data transfer from the source document to paper CRF are common<\/li><li>eCRF &#8211; data is obtained much quickly, resulting in timely database lock, faster regulatory submission, and subsequent approval<\/li><li>Despite their many advantages, eCRFs have not been accepted widely<\/li><li>lack of available on-site technology, investigators\u2019 lack of motivation, complexity of installation, and maintenance of the software and high investment cost<\/li><\/ul>\n\n\n\n<p><strong>Risk Based Monitoring<\/strong><\/p>\n\n\n\n<ul><li>An adaptive approach to clinical trial monitoring that directs monitoring focus and activities to the evolving areas of greatest need which have the most potential to impact patient safety and data quality.<\/li><li>Smarter use of Technologies that enable effective oversight<\/li><li><strong>Focus on what matters<\/strong><\/li><li><strong>Identify the risks; outliers<\/li><li>Identify higher risk sites to target additional monitoring<\/li><li>Ensure routine review of data in near real time<\/li><\/ul>\n\n\n\n<p><strong>Pharmacovigilance<\/strong><\/p>\n\n\n\n<p><strong><em>Mobile apps<\/em><\/strong>: These apps can enable pharma companies to collect ADR data much more quickly.<\/p>\n\n\n\n<p><strong><em>Cloud-based solutions<\/em><\/strong>: Cloud-based databases can enable pharma companies to collect data from multiple stakeholders to build an integrated ADR repository \u2013 even at a global level.<\/p>\n\n\n\n<p><strong><em>Artificial intelligence (AI):<\/em><\/strong>&nbsp;AI can help pharma companies to move beyond basic automation by identifying patterns in unstructured data.<\/p>\n\n\n\n<p><strong><em>Automation:<\/em><\/strong>&nbsp;RPA solutions can help pharma companies process structured data much more rapidly than via manual efforts.<\/p>\n\n\n\n<p><strong><em>Big data analytics:<\/em><\/strong>&nbsp;Analytics can help pharma companies use the vast amount of digital data available on the Internet to supplement traditional data sources such as primary calls, EHR data, and claims data for AE detection.<\/p>\n\n\n\n<p><strong><em>Proactive PV<\/em><\/strong>: Robust IT solutions and advanced systems can help pharma companies monitor drug safety during the research and trials process and post-launch.<\/p>\n\n\n\n<p><strong>Big data analytics<\/strong><\/p>\n\n\n\n<ul><li>The healthcare industry historically has generated large amounts of data, driven by record keeping, compliance &amp;
