{"id":749,"date":"2020-02-17T21:40:58","date_gmt":"2020-02-17T16:10:58","guid":{"rendered":"https:\/\/medicineplexus.com\/?p=749"},"modified":"2020-02-17T21:40:58","modified_gmt":"2020-02-17T16:10:58","slug":"pharmacovigilance","status":"publish","type":"post","link":"https:\/\/medicineplexus.com\/pharmacovigilance\/","title":{"rendered":"Pharmacovigilance"},"content":{"rendered":"\n<p><strong><u>Pharmacovigilance<\/u><\/strong><\/p>\n\n\n\n<p><strong><u>The etymological root<\/u><\/strong>: <em>pharmakon<\/em> (Greek) means drug and <em>vigilare<\/em> (Latin) means to keep awake or alert, to keep watch.<\/p>\n\n\n\n<p><strong><u>WHO Definition:<\/u><\/strong> The science and activities relating to the detection, assessment, understanding and prevention of adverse effects or any other drug related problems.<\/p>\n\n\n\n<p><strong><u>Why <\/u><\/strong><strong><u>Pharmacovigilance?<\/u><\/strong><\/p>\n\n\n\n<ul><li>Safety data from pre-clinical studies not reliable<\/li><li>Clinical trials are limited in time and number of patients<\/li><li>Rare or delayed serious reactions are likely to remain unnoticed.<\/li><li>To promote rational use of medicines<\/li><\/ul>\n\n\n\n<p><strong><u>Disasters:<\/u><\/strong><\/p>\n\n\n\n<ul><li>Thalidomide in 1962<\/li><li>Rofecoxib-Voluntary withdrawal (Sept 2004)<\/li><li>Rosiglitazone \u2013 banned in 2010<\/li><\/ul>\n\n\n\n<p>&nbsp;&nbsp; <strong>Incidence of hospital admissions related to adverse drug reactions<\/strong><strong><\/strong><\/p>\n\n\n\n<ul><li>In Australia between 2.4% and 3.6%<\/li><li>In United States between 3.1% and 6.2%&nbsp;<\/li><li>In India \u2013 data not available<\/li><\/ul>\n\n\n\n<p>&nbsp;&nbsp; Hence is responsible for huge expenditure on health.<\/p>\n\n\n\n<p><strong><u>Aims of the Pharmacovigilance:<\/u><\/strong><\/p>\n\n\n\n<ul><li><strong>Contribute to the regulatory assessment<\/strong> of benefit, harm, effectiveness and risk of medicines, encouraging their safe, rational and more effective (including cost effective) use.<\/li><li><strong>Improve patient care and safety<\/strong> in relation to use of medicines and all medical and paramedical interventions.<\/li><li>early detection of unknown safety problems<\/li><li>detection of increases in frequency<\/li><li>identification of risk factors<\/li><li>quantifying risks<\/li><li>communicating information<\/li><li>preventing patients from being affected unnecessarily<\/li><\/ul>\n\n\n\n<p><strong><u>Terms in Pharmacovogilance<\/u><\/strong><\/p>\n\n\n\n<ul><li><strong><u>Side Effect:<\/u><\/strong> Any unintended effect of a pharmaceutical product occurring at doses normally used in man which is related to the pharmacological properties of the drug. e.g. sedation with anti-histaminics.<\/li><li><strong><u>Adverse Event \/ Adverse Experience:<\/u><\/strong> Any untoward medical occurrence that may present during treatment with a pharmaceutical product at the same time, does not necessarily have a causal relationship with this treatment<\/li><li><strong><u>Adverse Reaction:<\/u><\/strong> A response to a drug which is noxious and unintended, and which occurs at doses normally used in man for the prophylaxis, diagnosis, or therapy of disease, or for the modification of physiological function<\/li><li><strong><u>Unexpected Adverse Reaction:<\/u><\/strong> An adverse reaction nature and severity of which is <strong>not consistent with domestic labelling or market authorisation<\/strong>, or expected from characteristics of drug action.<strong><u><\/u><\/strong><\/li><li><strong><u>Serious Adverse Event or Reaction<\/u><\/strong><\/li><\/ul>\n\n\n\n<p>A serious adverse event or reaction is any untoward medical occurrence that at any dose:<\/p>\n\n\n\n<ul><li>results in death<\/li><li>requires inpatient hospitalisation or prolongation of existing hospitalisation&nbsp;<\/li><li>results in persistent or significant disability\/incapacity<\/li><li>is life-threatening<\/li><li>Congenital anomalies<\/li><\/ul>\n\n\n\n<p><strong>Individual Case Safety Report (ICSR)<\/strong><\/p>\n\n\n\n<p>A report that contains \u2018information describing a suspected adverse drug reaction related to the administration of one or more medicinal products to an individual patient\u2019<\/p>\n\n\n\n<p><strong>Causality assessment<\/strong><\/p>\n\n\n\n<p>The evaluation of the likelihood that a medicine was the causative agent of an observed adverse reaction. Causality assessment is usually made according established algorithms.<\/p>\n\n\n\n<p><strong>SUSAR (Suspected Unexpected Serious Adverse Reaction):<\/strong><\/p>\n\n\n\n<p>Serious adverse drug reaction (SAR) that is unexpected or for which the development is uncommon (unexpected issue) observed during a clinical trial and for which there is a relationship with the experimental drug, whatever the tested drug or its comparator.<\/p>\n\n\n\n<p><strong><u>WHO Programme for International Drug Monitoring:<\/u><\/strong><\/p>\n\n\n\n<ul><li>WHO Drug Monitoring Programme was established in 1968<\/li><li>As of October 2013, 117 countries have joined the WHO Programme for International Drug Monitoring and 30 &#8216;associate members&#8217; are awaiting full membership.<\/li><\/ul>\n\n\n\n<p><strong><u>It consists of a network of the<\/u><\/strong><\/p>\n\n\n\n<p>&nbsp;&nbsp;&nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;1. National Centers,<\/p>\n\n\n\n<p>&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;2. WHO Headquarters and<\/p>\n\n\n\n<p>&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; 3. Uppsala Monitoring Centre which is WHO Collaborating Centre for International Drug Monitoring, in Uppsala, Sweden.<\/p>\n\n\n\n<p><strong><u>Functions:<\/u><\/strong><\/p>\n\n\n\n<ul><li><strong>Identification and analysis of new adverse reaction signal<\/strong>.<\/li><li>Provision of the <strong>WHO database.<\/strong><\/li><li><strong>Information exchange<\/strong> between WHO and National Centers mainly through Vigimed.<\/li><li>Publication of periodical newsletters, guidelines and books.<\/li><li>Provision of training and consultancy support to National Centres.<\/li><li><strong>Annual meetings<\/strong> for representatives of National Centres.<\/li><li>Producing WHO Drug Dictionary and the WHO Adverse Reaction Terminology.<\/li><li>Development of pharmacovigilance as a science.<\/li><\/ul>\n\n\n\n<p><strong><u>Pharmacovigilance Programme of India (PvPI):<\/u><\/strong><\/p>\n\n\n\n<ul><li>India joined the WHO Programme in 1998<\/li><li>The Central Drugs Standard Control Organization (<strong>CDSCO) launched the National Pharmacovigilance
