{"id":836,"date":"2020-02-17T22:13:38","date_gmt":"2020-02-17T16:43:38","guid":{"rendered":"https:\/\/medicineplexus.com\/?p=836"},"modified":"2020-02-17T22:13:38","modified_gmt":"2020-02-17T16:43:38","slug":"the-role-of-corticosteroids-in-dermatology","status":"publish","type":"post","link":"https:\/\/medicineplexus.com\/the-role-of-corticosteroids-in-dermatology\/","title":{"rendered":"The role of corticosteroids in dermatology"},"content":{"rendered":"\n<p><strong>The role of corticosteroids in dermatology<\/strong><\/p>\n\n\n\n<p><strong>Introduction<\/strong><\/p>\n\n\n\n<ul><li>Dermatology is one of the few disciplines in which we are able to apply therapy directly to the target site.<\/li><li>The concentration, the vehicle and the frequency of application can all be altered according to the response, which can easily be monitored.<\/li><\/ul>\n\n\n\n<ul><li>Corticosteroids have an important role because of their anti- inflammatory and immunosuppressive effects and also their anti-proliferative effects on keratinocytes.<\/li><\/ul>\n\n\n\n<ul><li>They can suppress collagen synthesis by fibroblasts, but this may lead to adverse effects. Conversely, this effect can help in the treatment of keloid scars.<\/li><\/ul>\n\n\n\n<p>Corticosteroids can also be given <strong>orally, parenterally or by injection into the lesion.<\/strong><\/p>\n\n\n\n<p><strong>Topical corticosteroids<\/strong><\/p>\n\n\n\n<p>The vasoconstrictor assay is the primary method of classifying the potency of topical steroids. This correlates with clinical efficacy.<\/p>\n\n\n\n<p>The efficacy and possible adverse effects depend on:<\/p>\n\n\n\n<p>&#8211; steroid type and the vehicle<\/p>\n\n\n\n<p>&#8211; application method &#8211; frequency, duration and use under occlusion<\/p>\n\n\n\n<p>&#8211; nature and extent of the skin disease<\/p>\n\n\n\n<p>&#8211; patient factors &#8211; age, site of the disease<\/p>\n\n\n\n<p>All these factors have to be taken into account to obtain the maximum benefit with the minimum adverse effects.<\/p>\n\n\n\n<p><strong>Skin disease<\/strong><\/p>\n\n\n\n<ul><li>In general, acute inflammatory eruptions respond well to mild\/moderate strength topical steroids (Class I and II).<\/li><li>Chronic, thickened or hyperkeratotic dermatoses may require potent or very potent steroids (Class III and IV).<\/li><\/ul>\n\n\n\n<p><strong>Vehicle<\/strong><\/p>\n\n\n\n<p>For a given strength of the same steroid, <strong>ointments are more potent<\/strong> than creams. The occlusive nature of ointment enhances steroid penetration. Ointment is often used for dry, fissured and lichenified skin disease because of its moisturising effect.<\/p>\n\n\n\n<p><strong>Creams (oil in water emulsions<\/strong>) may be <strong>drying <\/strong>and thus <strong>more suitable for acute and subacute weeping lesions<\/strong>. They are the <strong>most suitable vehicle for the moist and intertriginous (flexural) areas<\/strong>. Creams require the addition of emulsifiers and preservatives that have the potential to sensitise and cause allergic reactions.<\/p>\n\n\n\n<p><strong>For the scalp, steroids are often delivered in a lotion or gel<\/strong>. These vehicles lack the moisturising benefit of either creams or ointments. <strong>Propylene glycol can act as a penetration enhancer<\/strong>. By using it in a vehicle, the delivery of the topical steroid can be increased.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td><strong>A classification of the potency of commonly used topical corticosteroid preparations<\/strong><\/td><\/tr><tr><td><strong>Class I &#8211; mild<\/strong><\/td><td><\/td><\/tr><tr><td>hydrocortisone<\/td><td>0.5 &#8211; 1.0%<\/td><\/tr><tr><td>hydrocortisone acetate<\/td><td>0.5 &#8211; 1.0%<\/td><\/tr><tr><td><strong>Class II &#8211; moderate<\/strong><\/td><td><\/td><\/tr><tr><td>aclomethasone dipropionate<\/td><td>0.05%<\/td><\/tr><tr><td>betamethasone valerate<\/td><td>0.02%<\/td><\/tr><tr><td>betamethasone valerate<\/td><td>0.05%<\/td><\/tr><tr><td>triamcinolone acetonide<\/td><td>0.02%<\/td><\/tr><tr><td>triamcinolone acetonide<\/td><td>0.05%<\/td><\/tr><tr><td><strong>Class III &#8211; potent<\/strong><\/td><td><\/td><\/tr><tr><td>betamethasone dipropionate<\/td><td>0.05%<\/td><\/tr><tr><td>betamethasone valerate<\/td><td>0.1%<\/td><\/tr><tr><td>methylprednisolone aceponate<\/td><td>0.1%<\/td><\/tr><tr><td>mometasone furoate<\/td><td>0.1%<\/td><\/tr><tr><td><strong>Class IV &#8211; very potent<\/strong><\/td><td><\/td><\/tr><tr><td>betamethasone dipropionate<\/td><td>0.05% in an optimised vehicle<\/td><\/tr><tr><td>halcinonide<\/td><td>0.1%<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td>A guideline of cutaneous diseases for which corticosteroids are used, and the likely clinical response<\/td><\/tr><tr><td><strong><em>Most responsive<\/em><\/strong><strong><\/strong><\/td><td><strong><em>Less responsive<\/em><\/strong><strong><\/strong><\/td><td><strong><em>Least responsive<\/em><\/strong><strong><\/strong><\/td><\/tr><tr><td>Acute inflammatory disease including:<\/td><td>Psoriasis &#8211; plaque<\/td><td>Palmoplantar psoriasis<\/td><\/tr><tr><td>&#8211; allergic contact dermatitis<\/td><td>Seborrhoeic dermatitis<\/td><td>Discoid lupus erythematosus<\/td><\/tr><tr><td>&#8211; atopic eczema<\/td><td>Lichen simplex chronicus<\/td><td>Chronic hypertrophic lichen planus<\/td><\/tr><tr><td>&#8211; asteatotic eczema<\/td><td>Lichen planus<\/td><td>Granuloma annulare<\/td><\/tr><tr><td>&#8211; discoid eczema<\/td><td>Subacute cutaneous lupus erythematosus<\/td><td>Keloid scars<\/td><\/tr><tr><td>&#8211; napkin dermatitis<\/td><td>Papular urticaria (insect bite reactions)<\/td><td><\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p><strong>Occlusion<\/strong><\/p>\n\n\n\n<ul><li>The efficacy of steroids can be increased by application under occlusion.<\/li><li>This increases hydration of the skin and enhances penetration.<\/li><li>However, there is an increased risk of adverse effects if the use of steroid under occlusion is prolonged.<\/li><li>The occluding materials can include polythene gloves, plastic film (such as &#8216;Gladwrap&#8217;) and bio-occlusive dressings e.g. hydrocolloid.<\/li><\/ul>\n\n\n\n<p><strong>Region or sites<\/strong><\/p>\n\n\n\n<ul><li>The thickness of the stratum corneum,<\/li><li>local occlusive factors,<\/li><li>warmth and moisture are among the factors that can increase corticosteroid penetration and also the risk of adverse effects.<\/li><\/ul>\n\n\n\n<p><strong>As a rule, Class I steroids are preferred for the face and flexures<\/strong>. If a stronger corticosteroid is required, short-term (1-2 weeks) use is recommended.<\/p>\n\n\n\n<p>In contrast<strong>, palms and soles tend to require Class III or IV steroids<\/strong> as there is a thick stratum corneum and frequent accidental removal of the steroid may occur.<\/p>\n\n\n\n<p>Age<\/p>\n\n\n\n<p><strong>Premature babies<\/strong> and the elderly have relatively thin skin so <strong>steroid penetration is enhanced.<\/strong> The <strong>lower potency steroids<\/strong> are used first in this group. Particular care should be given when steroids are used in the nappy area.<\/p>\n\n\n\n<p>If treatment involves application to a large proportion of the body surface area, low- to medium-strength steroids are preferred because of the risk of extensive absorption.<\/p>\n\n\n\n<p>guidelines<\/p>\n\n\n\n<p>1. Topical steroids should be <strong>stopped when the skin disease has resolved<\/strong>. They are <strong>not used for prevention<\/strong> of disease.<\/p>\n\n\n\n<p>2. <strong>If possible, intermittent therapy<\/strong> is preferred to continuous application for long-term use. This is to <strong>reduce both tachyphylaxis and the risk of adverse effects.<\/strong><\/p>\n\n\n\n<p>3. <strong>Prolonged use should be avoided,<\/strong> where possible, on the face and the flexures.<\/p>\n\n\n\n<p>4. Some people benefit from using a more potent steroid first and then changing to a lower potency as the condition improves.<\/p>\n\n\n\n<p><strong>Adverse effects<\/strong><\/p>\n\n\n\n<p>In general, the greater the potency, the greater the risk of adverse effects.<\/p>\n\n\n\n<p>Skin atrophy, stellate scar and striae<br>Purpura and haemorrhage<br>Telangiectasia<br>Periocular, perioral dermatitis of the face<br>Folliculitis<br>Acneiform eruptions<br>Masking and aggravating fungal infections<br>Delayed wound healing<br>Hypertrichosis<br>Secondary infection and aggravating existing bacterial infections<\/p>\n\n\n\n<p><strong>Contact dermatitis<\/strong><br>&#8211; preservatives<br>&#8211; other ingredients in vehicles<br>&#8211; corticosteroid itself<\/p>\n\n\n\n<p>Systemic adverse effects may occur with the use of topical steroids. The risk factors include use in infants and children, prolonged and extensive use, use of high-potency steroids, use over large areas and use under occlusion. The adverse effects include the standard ones of adrenal suppression, growth retardation, Cushing&#8217;s syndrome and hypertension. Cataracts and glaucoma have been reported with periorbital use.<\/p>\n\n\n\n<p>Intralesional therapy<\/p>\n\n\n\n<p>The steroid is injected directly into the lesion. The common indications include recalcitrant lesions of <strong>nodular prurigo, keloid scars, acne cysts, discoid lupus erythematosus, hypertrophic lichen planus, alopecia areata and granuloma annulare.<\/strong><\/p>\n\n\n\n<p><strong>Triamcinolone acetonide is most often used<\/strong>.<\/p>\n\n\n\n<ul><li>It can be diluted with normal saline or lignocaine and is delivered in volumes of 1-2 mL.<\/li><li>The concentration used depends on the pathology and skin site.<\/li><li>Generally, one commences with a concentration of 5 mg\/mL working up to the full concentration of 40 mg\/mL, if necessary, when there is no response. If required, the injection can be repeated every 4-6 weeks.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td><\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>Systemic corticosteroids<\/p>\n\n\n\n<p>Steroids are required systemically for:<\/p>\n\n\n\n<p>&#8211; acute and chronic disabling bullous disorders including pemphigus vulgaris, bullous pemphigoid, pemphigoid gestationalis<\/p>\n\n\n\n<p>&#8211; connective tissue diseases &#8211; systemic lupus erythematosus, dermatomyositis, Wegener&#8217;s granulomatosis, relapsing polychondritis<\/p>\n\n\n\n<p>&#8211; acute steroid sensitive disorders, e.g. severe acute allergic contact dermatitis where the allergen is known and can be avoided to prevent relapse on cessation of steroids<\/p>\n\n\n\n<p>&#8211; severe, widespread, inflammatory atopic eczema where acute control is required (care is necessary on cessation of the oral steroids to prevent a widespread severe relapse)<\/p>\n\n\n\n<p>&#8211; others &#8211; severe lichen planus, urticaria, pyoderma gangrenosum, Sweet&#8217;s disease, Behcet&#8217;s and severe vasculitis<\/p>\n\n\n\n<p>Oral steroids are perfectly satisfactory in the majority of dermatology patients who are rarely ill enough to require either intramuscular or intravenous administration.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The role of corticosteroids in dermatology Introduction Dermatology is one of the few disciplines in which we are able to apply therapy directly to the target site.
